The following form is designed to gather appropriate details prior to joining the F.I.G. Programme. The questions are designed to assess the level of support that you may need, and to ensure that we have the appropriate structures in place to maintain your wellbeing and safety. 

Some of the questions may be difficult to answer, please take your time, make sure you take a break if you need, and feel free to call if you need to talk through any answers. Don’t feel as though you need to go into mountains of detail, but a nod to what may arise would help get those protective structures in place prior to attendance. 

As a part of data protection and GDPR, we will store your details safely and will keep them on file for 6 months following cessation of attendance. The details given on this form won’t be shared unless there is a safeguarding concern.
